To promote the health and well-being of clients by informing potential consumers about available services, how these services can meet the consumers needs, and encouraging consumers to seek medical care when needed. Serving as a Van Driver transportation services will be provided to patients of the Ben Archer Health Centers when appropriate.
Education and Experience Required:
High School graduate or GED. Prefer two years community outreach experience in a social services function. Prefer ten years driving experience with one-year experience as a driver.

General Job Description:
The Social Worker/LBSW is responsible for case management and outreach services to patients and clients of the health center.
Education and Experience Required:
Bachelors Degree in Social Work from an accredited college or university.
Must be licensed by the NM Board of Social Work Examiners.
Special Requirements:
Must have current NM Drivers License and show current proof of insurance.
Ability to speak English and Spanish.
Maintains appropriate confidentiality
